Position Summary
The Staff Nurse for Home Health / Eldercare Services and School Nursing is a dynamic and essential role within the Home Health Department of Petersburg Medical Center. This position is specifically tailored to support the expanding demands of our eldercare services. The role encompasses a broad range of responsibilities, designed to provide comprehensive nursing care and support to patients within their homes and the wider community. This position requires adaptability and a commitment to delivering high-quality care across various eldercare programs, including but not limited to:
  • Home Health Agency Nursing
  • Adult Day Program
  • Senior In-Home Services
  • Additional responsibilities as assigned.
This position requires dedication to promote and preserve the health, independence, and dignity of elderly patients, aligning with the mission, vision, and values of Petersburg Medical Center.

Responsibilities
  • Provide direct nursing care to patients in their homes, ensuring the delivery of safe, effective, and personalized treatment plans.
  • Collaborate with a multidisciplinary team to coordinate and implement comprehensive care plans that address the unique needs of patients.
  • Participate in the planning and delivery of services within the Adult Day Program and Senior In-Home Services Program, promoting high-quality eldercare.
  • Maintain accurate and detailed patient records, ensuring compliance with regulatory standards and departmental policies.
  • Demonstrate flexibility and adaptability, transitioning seamlessly between the different eldercare programs and responsibilities as required.
  • Engage with patient and their families to educate and support them in managing health conditions, making lifestyle adjustments and accessing community resources.
  • Uphold the mission, vision and values of Petersburg Medical Center acting as an ambassador for the Home Health Department and its services.
  • Receives patient referrals concerning the need for home care services.
  • Evaluates and updates client plan of care and coordinates services from time of admission to time of discharge.
  • Administers therapeutic treatments, pharmaceutical and parenteral solutions as ordered by client's primary care provider.
  • Maintains legal medical record of care planning, client progress, therapeutic and nursing interventions, and patient responses.
  • Assists the clinical manager in orientation to Home Health of new ancillary service providers.
  • Participates in regularly scheduled interdisciplinary team meetings, agency's Professional Advisory Committee, and assists with the orientation of new staff.
  • Functions within the limits of the Alaska State Nurse Practice Act, mission statement of Medical Center, and hospital and department policies. Follows professional code of ethics.
  • Maintains patient confidentiality per HIPAA and facility regulatory and / or policy guidelines on privacy.
  • Assist other staff (i.e., Patient Navigator) with case management of participants in all programs as needed.
  • Work collaboratively to develop eldercare services in the Petersburg Borough that may require state, community or federal grant program assistance.
  • Provide routine services as scheduled and as needed for the Cedar Social Club.
Qualifications / Education / Experience
Graduate of an accredited school of nursing as a Registered Nurse.
Experience. Two (2) years of clinical nursing experience, with at least one (1) of those years in an acute care of skilled nursing facility. Preferably one (1) year of public health or home health experience.
Licensing. Registered Nurse (RN) in the State of Alaska. Alaska state driver's license and insured personal vehicle required.
Employment Requirements include:
  • Background Check. Employment is contingent upon the successful completion of a background investigation conducted by the State of Alaska in accordance with applicable regulations.
  • Drug Screening. All employees must successfully complete a pre-employment drug screening, which includes testing for marijuana.
  • Immunization Compliance. Employees are required to meet all immunization requirements as mandated by the State of Alaska and Federal regulations governing healthcare facilities.
Working Conditions / Physical Requirements
Moderate to heaving lifting (no greater than 50 pounds), standing, walking and handling of clients, equipment and supplies. Work is primarily off-campus in the community and must have a high ability level to community effectively with manager, team and patients.

Environment
Variable. Client homes, Adult Day program location and other community locations as outlined by program needs.

Hazards
Moderate. May involve highway travel during hazardous weather conditions, management of behavior health patients, lifting and transferring of patients requiring hands-on support or with heavy care needs.
