Job Summary
The Home Health Certified Nurse Assistant (CNA) requires flexibility, innovation, critical thinking, and problem solving. The CNA duties would include but are not limited to working autonomously in the field with one-on-one patient care; supervised by Therapies or RNs every two weeks in the home. Required to meet all training and regulatory guidelines. The CNA would work to achieve advanced skills within the scope of practice guidelines. The CNA would work to achieve advanced skills within the practice of CNA by board of nursing including -- ability to help patient with their glucometers and report the results to RN, basic wound care, Remote Patient Monitoring, and home exercise programs as outlined by therapies. The CNA is an integral part of the Home Health team.

Responsibilities
  • Performs all the functions according to the established policies, procedures, and regulatory requirements as well as applicable professional standards. Provides all customers of Petersburg Medical Center with an excellent service experience by consistently demonstrating our Guiding Values and Code of Conduct.
  • Maintains confidentiality of client and hospital matters.
  • Presents the Home Health Agency and PMC in a positive manner.
  • Performs Home Health CNA tasks as assigned by Home Health manager / nurse / therapist.
  • Provides maximum client care services within the patient specific plan of care.
  • Treats clients in a manner conducive with their safety and comfort.
  • Accurately documents tasks performed daily consistent with patient plan of care.
  • Performs duties in accordance with established methods and techniques in conformance with recognized standards.
  • Recognizes symptoms indicative to adverse changes in patient status and reports unusual findings to the Nurse / Therapist.
  • Possesses a genuine interest in nursing and a willingness to work with the realization that errors may have serious consequences for patients.
  • Receives teaching and direction from supervisor without difficulty and maintains excellent rapport with therapists, nursing staff, patients and families.
Qualifications
Graduate of an accredited program for Certified Nurse Assistant in the State of Alaska, high school diploma and some college preferred.
Licensing - must be a CNA in the State of Alaska.
Experience - one year of CNA experience within a hospital, nursing home or home health agency with some geriatric focus.
  • Alaska state driver's license and an insured personal vehicle.
  • Cell phone that is in working order during office hours and computer competency.
  • Must demonstrate organizational abilities and flexibility.
  • Must show excellent critical thinking skills for autonomy, communication techniques and an exceptional demonstration of independent work.
  • Must possess ability to work and manage multiple projects and activities simultaneously.
  • Must be able to pass a State of Alaska Background Check and drug screen (including marijuana).
Working Conditions
Physical Requirements. Moderate to heavy lifting (no greater than 50 pounds) standing, walking and handling of patients, equipment and supplies.
Environment. Variable. Patient homes, office work and remote work.
Hazards. Moderate. May involve driving and walking outside during hazardous weather conditions.
