Clinic Nurse and Case Manager

Reports To
Clinic Manager

Position Summary
The Clinic Nurse plays a vital role in beam-based primary care, providing direct patient care and case management support to patients, families, and the healthcare team. This role focuses on improving patient health through education, prevention, and risk reduction. Strong communication skills are essential for coordinating care between patients, families, providers, and specialists.

Key Responsibilities
  • Provide advanced clinical nursing care in an outpatient setting, including wound care, administering medications, treatments, dressings, catheters and vaccines, as well as assisting with procedures as directed by the medical provider.
  • Provide case management for prenatal, postpartum, and pediatric patients.
  • Support Chronic Care Case Management, Transitional Care Management and Remote Home Monitoring as needed.
  • Collect patient history, vital signs, and medication details, reporting to providers.
  • Record patient interactions, facilitate medication refills, and process prior authorizations.
  • Assist patients with essential medical paperwork and forms.
  • Provide patient education based on condition and provider's recommendations.
  • Perform nursing assessments and triage within scope of practice.
  • Foster teamwork and maintain respectful interactions with staff, providers, and patients.
  • Utilize Lippincott procedures per policy and stay informed on available patient resources.
  • Collaborate with and support the Lead RN Case Manager.
  • Follow Alaska State Nurse Practice Act guidelines and maintain required licensure / certifications.
  • Mentor team members as needed.
  • Perform other duties as assigned.

Required Knowledge
All employees should have a general understanding of standard office techniques, including computers, printers, electronic health records, report analysis, and common office software.

Job Specific Knowledge
  • Proficiency in clinical nursing care, including wound care, medication administration, catheter management and assisting with medical procedures.
  • Familiarity with obstetrics, pediatrics, and chronic disease management is beneficial.
  • Knowledge of case management principles and patient-centered care is a plus.
  • Experience supporting patients and their families in challenging situations.

Qualifications
Education. Registered Nurse (RN), associate or bachelor's degree with a minimum of two (2) years' experience preferred. State of Alaska licensure. BLS required, with ACLS and PALS within one year of hire.
  • Must be able to pass a State of Alaska background check.
  • Must be able to pass a drug screen (including Marijuana).
